In India, we have names in different format.
1. First name
2. Middle name
3. Surname name
When we fill in Passport Application form , They ask for
1.Given name (First name)
2. Surname ( Father's name or Family Name or Caste name)

When we fill Indian Income tax -PAN Permanent Account Number form 49AA.

1. Surname or Last name( Father or Family Or Caste name)
If you don't have any name of the above format, fill in your first name as Surname.
Since, It is mandatory to fill in. This first letter is taken as the 5 th letter in your PAN card.

· How should I fill my first name and last name in a form, when I have my surname before my first name in India?
· There is no single system of describing one's name properly in India.
· The system that the British administrators introduced was
· 1. Given name,
· 2 . Father's name and
· 3. Surname.
· T hey were to be used as first name, second name and third name respectively.
· For example: Mohandas Karmachand Gandhi.
· I think this should be introduced as the procedure in all schools for every child throughout India.
· Confusion has arisen because of different practices of writing one's name in different places and cultures.
· 1.Some one writes his surname in full and uses initials for his family name and father's name. (B.N.Vijayakumar)
· 2.Some one uses only the family name as initial. (B.Vijay Kumar )
· 3. Some one does not use any initials at all. (Vijay Kumar)

· (Once, I had three colleagues in the same office - B.N.Vijayakumar, B.Vijay Kumar and Vijay Kumar. It had created a lot of confusion in identifying the right Vijay Kumar for the right purpose.)
· There is also a confusion in names of girls. Along with their given name,
· 1.they have their father's name and surname before marriage
· but
· 2.adopt husband's name and his surname after marriage.
· This practice has been officially accepted by the government, but without a set procedure or authentic stamp of approval.
· The present legal procedure for change of name of any person, man or woman, or otherwise, includes a sworn affidavit before a notary public and news paper advertisement. But, can every one afford to do it?
· I I think the Indian parliament should discuss this issue threadbare and adopt a common procedure.
